Repurposing dichloroacetate for the treatment of women with endometriosis

The study compared metabolic phenotypes of human peritoneal endometriosis (Endo)–derived human primary endometrial stromal cells (HPMCs) versus cells from women without endometriosis, using Seahorse glycolytic stress tests, lactate measurements, and PDH activity assays, and then tested whether dichloroacetate (DCA) could reverse the differences. Endo HPMCs showed higher basal glycolysis and lactate secretion with lower ATP-linked mitochondrial respiration and decreased PDH enzymatic activity, and treatment with DCA normalized these measures both with and without TGF-β1, also reducing lactate release and blocking TGF-β1–induced stromal cell proliferation in a lesion-in-a-dish coculture system. In a mouse endometriosis model, oral DCA for 7 days reduced peritoneal fluid lactate concentrations and decreased endometriosis lesion size.
