The Association between Endometriosis, Tubal Ligation, Hysterectomy and Epithelial Ovarian Cancer: Meta-Analyses

This meta-analysis of 38 case-control publications (40,609 EOC cases and 368,452 controls; studies from 11 countries, 1988–2016) investigated whether endometriosis, hysterectomy, or tubal ligation are associated with epithelial ovarian cancer, using pooled odds ratios with random-effects models and assessing heterogeneity and publication bias. The authors found a statistically significant association between endometriosis and increased EOC risk (OR = 1.42, 95% CI 1.28–1.57), while hysterectomy showed no association with EOC risk (OR = 0.97, 95% CI 0.81–1.14), and tubal ligation was associated with decreased EOC risk overall (OR = 0.70, 95% CI 0.60–0.78; with no association in Asia). They report high heterogeneity across studies for each exposure and limited analyses to case-control evidence, even though prospective cohort data exist, which they note as a rationale for their design choice.
