In vitrofertilization outcomes in women with surgery induced diminished ovarian reserve after endometrioma operation: Comparison with diminished ovarian reserve without ovarian surgery

This retrospective study compared fresh IVF outcomes in women under 40 with diminished ovarian reserve (DOR) defined by AMH <1.1 ng/mL or AFC ≤6, contrasting DOR after endometrioma surgery (endometrioma cystectomy or unilateral oophorectomy; n=32 women, 47 cycles) versus DOR without ovarian surgery (n=92 women, 119 cycles; current endometrioma excluded but other benign cysts allowed). Across groups, baseline stimulation parameters such as AFC, AMH, gonadotropin dose, and stimulation duration were similar, and key IVF laboratory outcomes (cycle cancellation, fertilization rate, mature oocytes, and top-quality embryo rate) did not differ significantly. Clinical pregnancy and live birth rates were numerically lower after endometrioma operation but were not statistically different, with age differing between groups and acknowledged limitations of small sample size and retrospective design.
