Peritoneal fluid and serum leptin concentrations in women with primary infertility

This prospective study measured leptin concentrations in serum (before surgery) and peritoneal fluid (collected during laparoscopy) in women undergoing laparoscopy as part of the diagnostic workup for primary infertility between 2005 and 2007, including groups with unexplained infertility, PCOS, bilateral tubal occlusion, and endometriosis. Women with PCOS had higher body weight, BMI, and serum leptin, while peritoneal fluid leptin was significantly higher in the endometriosis group than in the other groups and correlated positively with endometriosis stage; serum leptin did not relate to disease extent. The authors note leptin’s proposed roles in reproductive function and suggest peritoneal leptin involvement in the pathogenesis of PCOS and endometriosis, with leptin’s inflammatory/angiogenic properties potentially linking to implant development.
