Abstract

Progestins have a valuable role in the treatment of the painful symptoms associated with endometriosis and should be considered a first-line therapy. They are comparable in efficacy to other medical approaches, such as estrogen-containing preparations and gonadotropin releasing hormone agonists, and have minimal adverse effects. In particular, progestins should be considered when it is necessary to avoid the effects of estrogen, or in women who do not need, or want, to use contraception. Progestins are available in many forms, including oral preparations, injections, subdermal implants and intrauterine systems. The levonorgestrel intrauterine system, delivering high-dose progestin with few systemic side-effects, is a particularly promising long-term management option for endometriosis. The mechanism of action of progestins in the treatment of painful symptoms of endometriosis is currently not known.
