Altered Gene Expression Profiling in Endometrium: Evidence for Progesterone Resistance
This paper reviews evidence for progesterone resistance in the endometrium of women with endometriosis, PCOS, and endometrial hyperplasia and identifies potential mechanisms contributing to reduced progesterone activity.

This review analyzes evidence that endometrial progesterone resistance exists in women with endometriosis, PCOS, and endometrial hyperplasia, focusing on altered expression of progesterone-regulated genes and underlying mechanisms such as reduced progesterone activity (including cAMP-related pathways). It synthesizes prior gene-expression, receptor, and epigenetic findings—including work on altered progesterone receptor isoform expression and promoter hypermethylation—to propose potential explanations for dysregulated progesterone signaling in subsets of women. A key caveat acknowledged is that the genesis and consequences of progesterone resistance remain unclear, and existing studies do not fully resolve how these molecular alterations translate into functional outcomes. This paper is centrally about endometriosis — it reviews progesterone resistance in eutopic endometrium and highlights mechanisms linking impaired progesterone signaling to endometriosis.
