Primiparous singleton women with endometriosis have an increased risk of preterm birth: Meta-analyses
This meta-analysis of six studies found that women with endometriosis have an increased risk of preterm birth compared to those without.

This paper performed a systematic review and meta-analysis of observational cohort studies (PubMed, Medline, KERIS, Scopus, and Google Scholar from March 1994 to February 2016) to assess whether primiparous singleton pregnant women with endometriosis have an increased risk of preterm birth, using random-effects models and risk-of-bias assessment for non-randomized studies. Across six eligible studies totaling 50,472 patients, women with endometriosis had a significantly higher risk of preterm birth (odds ratio 1.473, 95% CI 1.216–1.785), with publication bias judged unlikely based on funnel plot and statistical tests, and mild heterogeneity reported. The authors’ limitations include residual confounding because ART use was not adjusted in the meta-analysis, and several included studies varied in diagnostic certainty, timing of diagnosis relative to reproduction, and details on surgical treatment and timing between surgery and first pregnancy. This paper is centrally about endometriosis — it meta-analyzes whether endometriosis in primiparous singleton pregnancies increases the risk of preterm birth.
