Abstract

Endometriosis is a common and complex disease characterized by the presence of endometrial tissue outside the uterine cavity, which leads to multiple clinical manifestations, including chronic pelvic pain, dysmenorrhea, and infertility. This article discusses the pathogenesis, risk factors, symptoms, and modern approaches to the diagnosis and treatment of endometriosis. Various theories explaining the development of the disease are discussed. In addition, the prevalence of endometriosis among women of reproductive age and its impact on quality of life is analyzed. Special attention is paid to diagnostic methods, including laparoscopy and imaging, as well as drug and surgical treatment strategies. The article highlights the need of raising awareness of the disease, early detection, and implementation of an integrated approach to endometriosis management. The article aims to enhance understanding of this condition and find new therapeutic solutions contributing to improved quality of life of women suffering from endometriosis.

Эндометриоз — патологический процесс, при котором определяется наличие ткани по морфологическим и функциональным свойствам подобной эндометрию вне полости матки [1]. Точная распространенность эндометриоза среди женщин неизвестна: она составляет 4 % среди фертильных женщин и 13,5 % среди бесплодных. У женщин, проходящих лапароскопию по поводу хронической тазовой боли, распространенность варьируется от 15 до 75 %. В 2015 году в Северной Индии оценили распространенность от 2 до 10 % среди общего населения, до 50 % среди бесплодных женщин. Эндометриоз затрагивает около 70 % женщин с дисменореей и диспареунией. Глобальные оценки распространенности колеблются от 2 до 45 %, при этом мировая статистика указывает на 10–15 % женщин с этим заболеванием [2]. К факторам риска развития эндометриоза относятся: раннее менархе (до 11 лет), короткие менструальные циклы (менее 27 дней), низкий индекс массы тела, кавказская национальность, возраст 25–29 лет и ежедневное употребление алкоголя (не менее 10 г в день). Эндометриоз чаще диагностируется у бесплодных женщин, активно курящих и с нормальным или низким ИМТ. Диета также играет важную роль: красное мясо, содержащее диоксины и жиры, может повышать риск эндометриоза. Исследование показало, что женщины, потребляющие более 2 порций красного мяса в день, имеют на 56 % более высокий риск эндометриоза по сравнению с теми, кто ест менее 1 порции в неделю. Не было установлено связи между потреблением птицы, рыбы, моллюсков и яиц с риском эндометриоза [7]. Ведение активного образа жизни с достаточным отдыхом и физической активностью также важно для профилактики первичного эндометриоза [8].
