Explorative laparotomy found a strictured terminal ileal segment with associated regional lymph node involvement and a concurrent 2-cm left ovarian endometrial cyst; resection histopathology confirmed ileal endometriosis with endometrioid glands and stroma in the muscularis propria and ER/PR-positive (CD10-positive) stromal cells, while the mucosa remained intact. The authors emphasize that preoperative differentiation of ileal endometriosis from Crohn’s and other terminal ileum conditions is difficult because imaging and endoscopic biopsies may be limited, and the findings are based on a single case without consensus on lymph node management.
