Abstract

This paper reviews present knowledge and future research needs regarding the relationship between psychosocial factors and endometriosis. Two central areas are reviewed: psychosocial factors which research suggests may be implicated in the etiology of endometriosis; and the psychological impact of the disease. The general dearth of data from population-based, longitudinal or other substantive studies means that the results of many studies should be treated with considerable caution. In particular, studies seeking to investigate possible psychological causes by comparing patients with endometriosis with other populations are misleading given the probable psychological impact of endometriosis on the woman concerned. There is more general agreement that endometriosis can be emotionally debilitating and future research could be usefully directed towards a clearer understanding of the psychological impact of the disease, coping resources and counseling needs.Die vorliegende Abhandlung befaßt sich mit dem gegenwärtigen Stand der Erkenntnis und der Notwendigkeit weiterer Forschungen im Hinblick auf Zusammenhänge zwischen psychosozialen Faktoren und dem Auftreten von Endometriose. Zwei Themen stehen im Mittelpunkt der Betrachtungen: Zum einen geht es um psychosoziale Faktoren, von denen aufgrund von Forschungsergebnissen angenommen wird, daß sic in der Ätiologie der Endometriose eine Rolle spielen könnten; zum anderen um die psychischen Auswirkungen dieser Erkrankung. Da ein genercller Mangel an Angaben besteht, die aus populations-gebundenen Längsschnitts- oder anderen aussagekräftigen Studien stammen, sind die Ergebnisse vieler Arbeiten mit größter Vorsicht zu behandeln. In Anbetracht der psychischen Folgen, die die Endometriose bei den Betxoffenen hervorrufen kann, sind besonders jene Unter-suchungen irreführend, bei denen mögliche psychologische Ursachen der Erkrankung anhand eines Vergleichs zwischen Endometriose-Patientinnen und Frauen aus anderen Populationen aufgedeckt werden sollen. Weitgehend einig ist man sich jedoch darin, daß die Endometriose ein starke emotionale Belastung darstellt und daß weitere Forschungsar-beiten daher zweckmäßigerweise darauf gerichtet sein sollten, ein besseres Verstandnis der psychischen Auswirkungen dieser Erkrankung zu erlangen, Möglichkeiten der seelischen Bewältigung aufzudecken und ein ausreichende Beratung zu sichern.Cet article fait le point sur les connaissances actuelles et sur les besoins futurs en recherche concemant la relation entre les facteurs psychosociaux et l'endométriose. Les auteurs se penchent sur deux secteurs principaux: les facteurs psychosociaux qui, comme l'indiquent les recherches, peuvent ětre impliqués dans l'étiologie de l'endométriose et l'impact psychologique de cette maladie. Dans l'ensemble, le manque de données provenant d'études longitudinales basées sur la population, ou d'autres études autonomes, signifie que les résultats de bon nombre d'études ne doivent ětre traités qu'avec la plus grande circonspection. En particulier, les investigations portant sur d'éventuelles causes psychologiques, dans le cadre desquelles des patientes souffrant d'endométriose sont comparées à d'autres populations, prětent à confusion du fait de l'impact psychologique probable de l'endométriose sur les femmes atteintes. On convient plus généralement que l'endométriose peut ětre débilitante au point du vue émotif; dès lors, les recherches ultérieures pourraient utilement ětre orientees vers une comprehension plus complete de l'impact psychologique de cette maladie, du potentiel d'adaptation à celle-ci et du recours à des conseillères.
