This 2024 narrative review summarizes literature on complications of deep infiltrative endometriosis involving the gastrointestinal tract, with emphasis on ileocecal disease. Drawing on PubMed/Google Scholar publications, the authors describe reported cases and mechanisms for intestinal perforation, intestinal obstruction (including small bowel and ileocecal/angle-level obstruction), and bleeding, noting that pregnancy/postpartum timing and hormonal and inflammatory factors are associated with perforation. They report that only about 20 intestinal perforation cases are described in the literature and that evidence for obstruction is largely derived from case reports and heterogeneous surgical series, with a major caveat that symptoms and intraoperative appearance can mimic malignancy.
