This case report describes a 63-year-old post-menopausal woman with new-onset altered bowel habits and lower abdominal pain who developed acute recto-sigmoid large bowel obstruction; investigators attempted colonoscopy but stricturing prevented progress, and CT suggested an underlying malignancy. Emergency laparotomy found a large rectosigmoid mass with adhesions to nearby small bowel, omentum, and the pelvic brim, and a Hartmann’s procedure was performed, after which she recovered without complication. Histopathology revealed extensive vascularized adhesions and endometriosis in the wall of the recto-sigmoid junction, with no evidence of colonic malignancy, and she later underwent reversal of Hartmann’s. The authors emphasize that pre-operative diagnosis is often impossible because mucosal biopsies can be normal and imaging may only show extrinsic compression, making this a diagnostic-limitation case report rather than generalizable research.
