Este artículo presenta un reporte de caso en una mujer posmenopáusica de 71 años con dolor abdominal y cambios en el hábito evacuatorio, que ingresó por suboclusión intestinal aguda. Mediante imagen se observaron signos de obstrucción distal sin aire, y en laparotomía exploradora se identificó una lesión inflamatoria en rectosigma con compromiso del intestino delgado, realizándose cirugía de Hartmann. El estudio anatomopatológico confirmó pared colónica con focos de endometriosis y adherencias a parénquima ovárico y tubárico. La conclusión del trabajo es que la endometriosis colónica puede presentarse como obstrucción e imita otras causas como enfermedad de Crohn o neoplasias, con diagnóstico preoperatorio dificultoso y necesidad de confirmación histológica.
