Small bowel obstruction due to an endometriotic ileal stricture with associated appendiceal endometriosis: A case report and systematic review of the literature

This paper presents a case report in a 44-year-old woman with recurrent subacute small bowel obstruction symptoms initially suspected to be Crohn’s disease, plus a systematic review of previously reported cases of endometriosis causing bowel obstruction. Clinic-radiologic evaluation showed distal ileal wall thickening with luminal narrowing and a tight terminal ileal stricture near the ileocecal junction, balloon dilation that failed quickly, consideration of tuberculosis, and ultimately diagnostic laparoscopy with laparoscopic right hemicolectomy. Histopathology demonstrated an endometrial origin with endometriotic foci in the ileum producing a fibroblastic secondary stricture and additional endometriotic deposits in the appendix, with estrogen/progesterone receptor positivity and CD10 staining in the stroma; the authors note a key caveat that preoperative diagnosis is difficult due to nonspecific symptoms and inconclusive imaging because endometriosis does not involve mucosa, limiting the usefulness of endoscopy.
