This review examines deep infiltrating endometriosis, a severe variant of the disease where endometriotic tissue penetrates deeply into pelvic organs and retroperitoneal spaces. The authors highlight significant diagnostic challenges due to nonspecific symptoms and limited sensitivity of standard methods, while noting that current treatments are largely symptomatic with high recurrence rates. To address these issues, the paper outlines emerging diagnostic tools such as molecular markers, high-resolution imaging, artificial intelligence algorithms, and standardized classification protocols. It also discusses potential therapeutic targets involving specific signaling pathways and immune mechanisms, alongside multidisciplinary rehabilitation strategies including physiotherapy and microbiota modulation. This paper is centrally about endometriosis — specifically deep infiltrating endometriosis (DIE) — covering its diagnosis, treatment, and rehabilitation.

Abstract

Deep infiltrating endometriosis (DIE) is a severe form of endometriosis characterized by deep infiltration of endometriotic tissue into the retroperitoneal space and muscular walls of pelvic organs. Its nonspecific clinical presentation and the limited sensitivity of standard diagnostic methods contribute to significant diagnostic delays, while current treatments are primarily symptomatic and associated with high recurrence rates. This review summarizes current and emerging approaches to the diagnosis of DIE, including molecular markers, high-resolution imaging techniques, standardized protocols and classifications, and artificial intelligence-based algorithms. Potential therapeutic targets involved in the pathogenesis of DIE are discussed, including the PI3K/Akt/mTOR signaling pathway, microRNAs, fibrogenic factors, and specific immune mechanisms. Particular attention is given to multidisciplinary rehabilitation strategies, such as physiotherapy, exercise, nutritional and psychological support, and modulation of the vaginal and gut microbiota. Further advances in the diagnosis, treatment, and rehabilitation of patients with DIE are needed to facilitate earlier diagnosis, enable personalized therapy, and improve long-term clinical outcomes.

Papers in the corpus that this work cites (lower rings, blue) and that cite this one (upper rings, green). Dot size scales with the paper's in-corpus citation count — bigger dot = more influential within the endo/adeno field. Click a dot to open that paper. Outer rings show 2-hop neighbours — papers reached through the immediate citers/citees. [ collapse to 1-hop ]
