Robotic-assisted laparoscopy has been used to treat bowel endometriosis with outcomes similar to standard laparoscopy, but long-term follow-up and larger sample sizes are needed.

The paper reviews evidence on robotic-assisted laparoscopy (RAL) for bowel endometriosis, focusing on feasibility and safety and covering techniques such as shaving, disc excision, and segmental colorectal resection for deep infiltrating and rectosigmoid endometriosis. It reports that surgical outcomes are similar between RAL and standard laparoscopy across prior studies, but it emphasizes that most supporting data are retrospective with small sample sizes and limited postoperative follow-up. A major limitation explicitly noted is the lack of investigations into long-term postoperative outcomes and the consequent inability to detect differences between treatment approaches. This paper is centrally about endometriosis — specifically robotic treatment of bowel endometriosis (including deep infiltrating and rectosigmoid disease).

Abstract

Over the last 10 years several studies demonstrated the feasibility and safety of robotic-assisted laparoscopy (RAL) in the treatment not only of deep infiltrating endometriosis but also of rectosigmoid endometriosis. RAL has been used to perform all the techniques used to treat bowel endometriosis: shaving, disc excision, and segmental colorectal resection. Similar outcomes have been reported in patients treated by RAL and standard laparoscopy. However, most of these studies are retrospective, they report the surgical outcomes and, sometimes, short-term postoperative follow-up. Unfortunately, long-term postoperative outcomes have not been investigated. In addition, the sample size of most of these studies is very small and, thus, insufficient to detect potential differences between groups.
