Clear Cell Adenocarcinoma Arising from Abdominal Wall Endometriosis

This case report describes a 49-year-old woman with a painful, large abdominal wall intramuscular mass that was initially clinically suspected to be a desmoid tumor and resected; pathology later showed clear cell adenocarcinoma arising in proximity to benign endometriotic foci in a surgical midline scar from a prior myomectomy. The tumor was characterized histologically by papillary/tubulocystic architecture with hobnail features and immunohistochemistry positive for vimentin, epithelial membrane antigen, and cytokeratin 7, with negative cytokeratin 20 and estrogen/progesterone receptors, and Sympson’s criteria were stated as fully satisfied. Despite positive surgical margins and subsequent attempted chemotherapy and additional surgery for recurrence with extension to the bladder and further progression, the report notes limited follow-up and that the chemotherapy regimen given later was not specified.
