Is Oocyte Quality Impaired in Cases With Ovarian Endometriosis? A Second Look Into the Clinical Setting
This opinion article re-examines the clinical evidence regarding potential impairment of oocyte quality in patients with ovarian endometriosis.

This paper is a narrative review of clinical evidence (PubMed searches from 2011–2021 plus manual cross-references) on whether oocyte quality is impaired in women with endometriosis undergoing ART, with emphasis on ovarian endometriosis/endometrioma, using outcomes such as oocyte morphology, morphokinetics, vitrification survival, embryology parameters, and pregnancy metrics. The authors report largely conflicting findings across retrospective controlled studies: one large study assessing oocyte morphology in 175 women with endometriosis found no difference in oocyte quality indices, while studies focused on endometrioma size, laterality, and prior surgery yielded mixed effects on maturation and blastulation, and time-lapse morphokinetics results are inconsistent, with some small studies showing no change in sibling oocytes. They explicitly note key limitations including the predominance of retrospective designs, confounding (especially endometrioma characteristics and ovarian surgery), and limited prospective data. Relevance to endometriosis: the paper’s central question is whether ovarian endometriosis/endometrioma affects oocyte quality and embryonic development in IVF/ICSI contexts.
