Abstract

鼠径部に発生する子宮内膜症は稀である．今回右鼠径部腫瘤を主訴とし，ヌック水瘤の診断で切除術を行ったところ水瘤内に子宮内膜症を認めた症例を経験したので報告する．症例は36歳未妊娠・未経産女性．右鼠径部に無痛性の腫瘤があるのに気づき来院．触診でヌック水瘤と診断し切除術を行った．右鼠径管内に長径3cm直径2cm大の卵型腫瘤を認めた．内鼠径輪から伸びる嚢状の腹膜組織と連続していたが内腔の交通は認めなかった．術中診断もヌック水瘤でありこれを切除した．腹膜鞘状突起腹膜側断端はメッシュプラグ法で修復した．病理組織検査でヌック水瘤内に子宮内膜と血液貯留を認め，ヌック水瘤内子宮内膜症と診断した．鼠径部の子宮内膜症は稀である．月経時の増大と疼痛を伴う腫瘤が特徴とされ，90％は右側に生じる．外科医・婦人科医はこれを念頭に外科的切除と腹腔内子宮内膜症の検索を行うことが望まれる．
