Abstract

症例は36歳の女性で5年程前より右鼠径部の腫瘤を自覚していたが放置(月経時に一致して大きくなり,疼痛も出現していた),腫瘤の増大と痛疼悪化のため当院を受診した.腫瘤は右鼠径部にあり20×15mm大で境界明瞭,弾性硬,可動性良好で圧痛を伴っていた.エコー上は右鼠径部腫瘤に一致して内部エコー不均一な低エコー病変として認められた.表面不整で境界も一部不明瞭であった.リンパ節炎あるいは子宮内膜症の疑いで,確定診断のため摘出術を行った.術中所見で外鼠径ヘルニアを認め,腫瘤はヘルニアサックの盲端に存在していた.周囲組織との癒着は軽度であり,腫瘤摘出,高位結紮術を行い手術を終了した.病理組織学的検査で摘出腫瘤は子宮内膜症であった.術後は特に問題なく経過し,約10カ月経過する現在に至るまで再発徴候は認めていない.本症例は外鼠径ヘルニアに併発した稀な症例であり,若干の文献的考察を加え報告する.
