Abstract

症例は36歳，女性．一年前に右足の付け根の膨隆に気づいた．容易に用手還納可能だったが，徐々に還納しづらくなったため当院受診．鼠径靭帯より尾側の大腿動脈近傍に膨隆を認め，右大腿ヘルニアの診断で手術目的に入院となった．腹腔鏡下ヘルニア根治術を施行．ヘルニア門は大腿輪で大腿ヘルニアの所見であり，囊胞性腫瘤が嵌頓していたため体外より用手的に還納した後，切除しヘルニア門を修復した．囊胞壁の病理組織学的検査では一部の領域において，腺管構造を認め，ER陽性を示す子宮内膜症病変と考えられた．子宮内膜症に合併する大腿ヘルニアは極めて稀であり，腹腔鏡手術を施行した症例は本邦初の報告例である．
