Abstract
症例は47歳,女性.十数年来自覚していた右鼠径部腫瘤の増大傾向を主訴に受診.右鼠径部に径9×8cmの弾性軟の腫瘤を認めた. CT・MRIなど画像検査で内部は液性成分を呈しており,腹腔内との交通は明らかでなかった.原発不明の皮下嚢胞の診断にて平成13年1月10日手術を施行した.手術時に腫瘤の大腿輪との交通を認め大腿ヘルニアと診断,ヘルニア嚢切除を伴うMesh Plugを用いたヘルニア根治術を行った.ヘルニア内容は腹水のみであった.切除したヘルニア嚢の病理組織学的検索にて子宮内膜症が確認された.子宮内膜症が疑われる鼠径部ヘルニア症例に対しヘルニア嚢切除を伴わない術式を選択した場合には,子宮内膜症が残存する可能性がある.子宮内膜症の併存が疑われる鼠径部ヘルニア症例に対してはヘルニア嚢の組織学的検索も考慮すべきではないかと考えられた.
