Abstract

患者は49歳の女性。3年前より右鼠径部に腫瘤を認め, ときおり疼痛も認めていた。右鼠径部の腫瘤に疼痛をきたし当院を受診, 同部に圧痛を伴い還納はできなかった。CT検査で低吸収域の腫瘤を認め, 右鼠径部腫瘤の診断で手術を施行した。腫瘤は外鼠径輪部にあり周囲組織と癒着していた。外鼠径ヘルニアを認め, ヘルニア嚢が腫瘤に連続し嚢を切開すると暗黒色血の流出を認めた。腫瘤は可及的に切除し, ヘルニア嚢は高位結紮, メッシュプラグ法で修復した。切除組織の病理組織検査で子宮内膜様組織を認めた。術後の病歴聴取で月経周期に一致した腫瘤の増大と疼痛があったことが判明した。鼠径部子宮内膜症は稀な疾患であるが, 鼠径ヘルニアに合併することがあり外科医が知っておくべき疾患と考えられる。
