Abstract
症例は26歳,女性.平成6年頃から右鼠径部腫瘤に気が付いていた.時々痛みがあるため,平成13年9月1日当科を初診した.右鼠径部には,母指頭大の境界明瞭な弾性硬の腫瘤を認めた.腫瘤に可動性はなく,圧痛を伴っていた.超音波検査では,大きさ1.8×1.6×0.9cmの表面不正で,内部エコー不均一の腫瘤を認めた. 10月15日,右鼠径部腫瘤の診断で手術を施行した.術中に外鼠径ヘルニアを認め,腫瘤はヘルニア嚢盲端に存在していた.ヘルニア嚢を高位結紮して腫瘤と共に切除し, Marcy法によるヘルニア修復術を施行した.腫瘤は肉眼的には肉芽組織で,茶褐色の液体を含む多胞構造を示していた.病理組織検査の結果,摘出腫瘤は子宮内膜症であった.術後約1年8カ月経過した現在,再発の徴候は認めていない.鼠径部子宮内膜症は,高率に鼠径ヘルニアを合併することが報告されており,日常診療において認識しておくべき重要な疾患であると思われた.
