Sphingosine I Phosphate (S1P) Increased IL-6 Expression and Cell Growth in Endometriotic Cells

The study examined whether the sphingosine 1-phosphate (S1P) signaling system contributes to endometriosis, by measuring sphingosine kinase 1 (SphK1) expression in endometriotic lesions (immunohistochemistry), quantifying S1P concentrations in cystic fluid from endometriomas versus nonendometriomas, and using endometrioma-derived endometriotic stromal cells (ESC) for in vitro analyses. SphK1 was detected in both epithelial and stromal components of lesions, cystic fluid S1P levels were higher in endometriomas (98.2 nM vs <1.5 nM), and cytokines IL-1β and transforming growth factor-β increased SphK1 mRNA in ESC. S1P increased ESC cell number and induced IL-6 mRNA and secretion in a dose-dependent manner, with receptor antagonists partially suppressing IL-6 induction (S1PR2) and ESC proliferation (S1PR1/S1PR3). A key limitation is that the evidence for mechanism and growth is based on ESC in vitro rather than in vivo models.
