Abstract

症例は44歳，女性．2017年３月と７月に腹痛・嘔吐を認め腸閉塞の診断で当院入院となった．腹部CTで回盲部の壁肥厚と小腸の拡張を認めた．症状が月経周期に一致して繰り返し出現したため回腸子宮内膜症が疑われた．月経困難症に対する，約６カ月間の偽閉経療法においては症状の再燃は認めなかった．内服中止後に再度腸閉塞を認め当院入院となり，骨盤MRIでは回腸にT1強調画像で高信号，T2強調画像では低信号の腫瘤性病変を認め，回腸子宮内膜症の診断となった．腹腔鏡下回盲部切除術を施行し，病理組織学的診断は回腸子宮内膜症であった．腸管傍リンパ節にも免疫染色で同様に子宮内膜組織像を認めた．術後経過は良好で第７病日に退院となった．
