Abstract

症例は生来健康な41歳の女性で，腹痛を主訴に救急外来を受診した．CTで回腸末端に壁肥厚と狭窄を認め，小腸閉塞の診断で入院加療を行った．入院中に精査を行ったが明らかな病変は同定できず，腫瘍や炎症性腸疾患は否定的であった．保存的加療により軽快し退院としたが，その後，再度腹痛を認め救急搬送された．CTで小腸閉塞および周囲にfree airを認め，穿孔性腹膜炎の診断で同日緊急手術を行った．手術は腹腔鏡補助下回盲部切除術を施行し，術後の病理組織診断で回腸子宮内膜症と診断された．術後は婦人科にて内分泌療法を行っており，現在まで再発を認めていない．今回われわれは，大変稀な回腸子宮内膜症による腸閉塞が原因で消化管穿孔をきたした1例を経験したので報告する．
