Abstract

症例は35歳女性．子宮内膜症で当院婦人科にて加療中に腸閉塞による突然の腹痛で入院となった．CT上，閉塞起点は回腸末端の壁肥厚であり，下部内視鏡観察では，同部位に圧排による腸管内腔の狭小化を認め，生検を行ったが確定診断はできなかった．悪性腫瘍の可能性も否定できないため，D3リンパ節郭清を伴う腹腔鏡下回盲部切除術並びに両側チョコレート囊胞摘出術を施行した．術中観察ではダグラス窩に子宮内膜症に特徴的な肉眼所見であるblue berry spotを多数認め，電気メスにて焼灼した．病理組織学的診断は，回腸粘膜下異所性子宮内膜症であり，リンパ節中にも同様の子宮内膜組織を認める他，腸管粘膜面にもblue berry spotが散見された．今回，腹腔鏡下手術により，異所性子宮内膜症による腸閉塞のみならず，多数の内膜症病変に対しても同時に加療を行った症例を経験したので若干の文献的考察を加え報告する．
