Abstract

症例は37歳，女性．性周期に一致した腹痛を主訴に産婦人科受診した．保存的治療にて改善せず，徐々に腹部膨満が出現した．注腸造影では，結腸には異常所見を認めなかった．その後，腹部単純写真にてniveauを認め当科紹介となった．入院後イレウス管を留置し，消化管減圧を行った．小腸造影を施行し，回腸末端に屈曲と狭窄を認めた．この部位が腸閉塞の原因と考え，開腹手術を施行した．回腸末端は屈曲し強固に癒着していた．硬結は触れるが，明らかな腫瘍性病変は指摘できなかった．術式は回盲部切除術を施行した．病理組織学的検査にて異所性子宮粘膜の増生を認め回腸子宮内膜症と診断した．術後経過は良好で，術後1年を経過した現在，再発無く経過中である．腸管子宮内膜症はそのほとんどがS状結腸や直腸に発生し，小腸に発生する頻度は7％と稀であるため今回報告する．
