Multimodality imaging and clinicopathologic assessment of abdominal wall endometriosis: knocking down the enigma

This retrospective study evaluated 116 women with histopathologically confirmed extragenital endometriosis (2/2014–6/2017), identifying 26 cases of abdominal wall endometriosis (AWE) and, in 18 of these, re-reviewing available ultrasound, CT, and/or MRI scans by two expert radiologists alongside clinical and pathology data from records. Imaging-identified AWE lesions were typically associated with prior C-section (15/18) and presented with abdominal pain (often cyclical), palpable mass, and sometimes skin discoloration or fluid discharge; pathology-imaged lesions (21 total) commonly showed stellate or round shapes, ill-defined heterogeneous appearances, ill-defined margins, involvement of deep and superficial wall layers, and variable modality-specific signal and vascular patterns, while original reports correctly diagnosing AWE in only 30.4% (7/23) of cases. The authors explicitly note diagnostic difficulty and a significant median delay from symptom onset to histopathology (24.41 months).
