Abstract

症例は71歳女性,既往歴に子宮内膜症と子宮筋腫に対する手術歴あり.当院内科で逆流性食道炎に対して加療中であった.スクリーニング目的で下部消化管内視鏡検査を施行したところ,盲腸に隆起性病変を認めた.病理生検にて悪性所見を認めなかったため経過観察としていたが,増大傾向にあり当科紹介となった.腹部造影CTにて盲腸内腔に突出した長径20mmの腫瘤を認めた.回結腸動脈沿いにリンパ節腫大を伴っていたため,悪性腫瘍も念頭に置き,腹腔鏡補助下回盲部切除術を行う方針とした.病理組織標本では盲腸内腔に反転した虫垂を認め,虫垂壁や盲腸壁内には子宮内膜腺を伴っており,虫垂子宮内膜症により生じた虫垂重積と診断した.経過は良好で術後10日目に退院した.
