This chapter reviews clinical evaluation and preoperative considerations for adolescent girls with endometriosis, emphasizing high-level diagnostic workup using history and physical examination and maintaining a strong index of suspicion. It describes a 6-month trial of NSAIDs and hormonal therapy before considering laparoscopy for definitive diagnosis and surgical management, noting that endometriosis can have variable intraoperative appearances that affect diagnostic confirmation. After confirmation and surgery by a skilled specialist, it outlines a recommendation for long-term hormonal therapy to reduce risk of recurrence and progression, with the caveat that the timing and diagnostic accuracy depend on clinical suspicion and surgical visualization/interpretation. This paper is centrally about endometriosis — it focuses on diagnosis, preoperative evaluation, and perioperative management considerations in adolescent girls with endometriosis.

Abstract

Endometriosis is an important cause of pelvic pain among adolescent girls. A thorough history, physical exam, and a strong index of suspicion are required for a timely diagnosis. A 6-month trial of NSAIDs and hormonal therapy are recommended prior to consideration of laparoscopy for a definitive diagnosis and surgical management of this chronic disease. An understanding of the variable appearance of endometriosis at the time of surgery is key to solidifying the diagnosis. Long-term hormonal therapy following confirmation and surgical management of endometriosis by a skilled gynecologist or pediatric surgeon is recommended to reduce risk of disease recurrence and progression.
