Abstract

症例は45歳,女性.下血を主訴に前医を受診し,注腸検査にて盲腸部腫瘍を指摘され,当院へ紹介受診となった.CTにて虫垂内腔に液体貯留を認め,虫垂粘液腫と診断した.虫垂粘液腫所見を呈する疾患として,非腫瘍性貯留嚢胞だけでなく,虫垂粘液嚢胞腺腫や虫垂粘液嚢胞腺癌も鑑別すべきであり,術前画像検査で腫瘍性嚢胞が否定できなかった.そのため,患者の同意のもと,腹腔鏡下回盲部切除術(D2)を施行し,術後病理組織学的検査にて,虫垂子宮内膜症の診断に至った.虫垂粘液腫の原因として,子宮内膜症も考慮すべきと考えられた.
