Abstract

症例は47歳，女性．残尿感，軽度月経困難を主訴に前医を受診し，MRIで子宮筋腫再発と虫垂粘液腫を疑う囊胞病変を認めたため，手術加療目的に当院に紹介となった．画像検査・内視鏡検査で虫垂粘液腫と診断し，子宮筋腫と同時に虫垂粘液腫に対して開腹手術を施行した．盲腸部分切除を施行し，術中迅速病理診断で悪性腫瘍が否定されたため，追加切除は行わなかった．免疫組織学的検査ではエストロゲンレセプター，CD10陽性で，虫垂子宮内膜症による粘液貯留と診断された．虫垂子宮内膜症は腸管子宮内膜症のうち約3％程度と言われており，頻度は少ない．また，虫垂粘液腫を呈するのはさらに稀で，鑑別疾患に挙がりづらい．今回われわれは，盲腸部分切除を施行した虫垂粘液腫の所見を呈した虫垂子宮内膜症の1例を経験したので，若干の文献的考察を加えて報告する．
