Abstract

症例は45歳，女性．下血を主訴に前医を受診し，注腸検査にて盲腸部腫瘍を指摘され，当院へ紹介受診となった．CTにて虫垂内腔に液体貯留を認め，虫垂粘液腫と診断した．虫垂粘液腫所見を呈する疾患として，非腫瘍性貯留嚢胞だけでなく，虫垂粘液嚢胞腺腫や虫垂粘液嚢胞腺癌も鑑別すべきであり，術前画像検査で腫瘍性嚢胞が否定できなかった．そのため，患者の同意のもと，腹腔鏡下回盲部切除術（D2）を施行し，術後病理組織学的検査にて，虫垂子宮内膜症の診断に至った．虫垂粘液腫の原因として，子宮内膜症も考慮すべきと考えられた．
