Abstract

症例は47歳，女性．便秘症の精査で施行した下部消化管内視鏡検査で，虫垂開口部に突出する腫瘤を認め，精査加療目的に当科紹介となった．組織生検では，悪性所見を認めなかった．腹部造影CTでは回盲部に造影効果を伴う充実性腫瘤を認め，虫垂腫瘍または虫垂子宮内膜症を疑い，待機的に腹腔鏡下手術の方針とした．術中所見では，虫垂は確認できず，盲腸内に虫垂が重積しており，虫垂のみの切除は困難なため，腹腔鏡下盲腸切除術を施行した．病理組織学的検査では，粘膜下層から漿膜下層にかけて類内膜腺管が点在する腸管子宮内膜症の像を呈しており，虫垂子宮内膜症と診断した．虫垂子宮内膜症による虫垂重積症は稀な病態であり，虫垂腫瘍との鑑別が困難である．今回，虫垂重積をきたした虫垂子宮内膜症に対して，腹腔鏡下盲腸切除術を施行した１例を経験したので，これまでの本邦報告例をまとめ，文献的考察を加えて報告する．
