This paper reports a clinical case of a 13-year-old nulligravida girl with continuous right lower abdominal pain, in whom an ovarian cyst found 1 month earlier was left untreated after an appendectomy. Menarche occurred 24 days before the current operation, and imaging and surgical findings led to a diagnosis of right hematosalpinx with peripheral tubal obstruction and a para-ovarian serous cyst; she underwent right salpingectomy with para-ovarian cyst resection, with normal-appearing ovaries and uterus on inspection. Histological examination confirmed hematosalpinx and tubal endometriosis, with the major limitation being that this is a single case report rather than systematic evidence. This paper is centrally about endometriosis — specifically diagnosing tubal endometriosis shortly after menarche in an adolescent.
