Removal of Endometriomas Before in vitro Fertilization Does Not Improve Fertility Outcomes: A Matched, Case-Control Study
This case-control study found that removing ovarian endometriomas before IVF did not improve fertilization, implantation, or pregnancy rates compared to proceeding directly to IVF.

This retrospective case-control study evaluated whether removing ovarian endometriomas prior to in vitro fertilization improves fertility outcomes compared to proceeding directly to treatment. The research analyzed 189 women with endometriosis, comparing 133 who underwent laparoscopic cystectomy against 56 who did not, while controlling for age and ovarian reserve. Results showed no significant differences between the groups in mature oocyte counts, fertilization rates, embryo numbers, or pregnancy and implantation rates. Although surgical removal altered gonadotropin dosage and estradiol levels, it did not enhance clinical success, suggesting that direct IVF may be a more efficient option for asymptomatic patients. This paper is centrally about endometriosis — specifically focusing on the management of ovarian endometriomas and their impact on assisted reproductive technology outcomes.
