Clinical presentation of perineal endometriosis and prognostic nomogram after surgical resection
This study characterized perineal endometriosis and developed a nomogram based on multiple lesions and positive surgical margins to predict recurrence after surgical resection.

This retrospective study evaluated the clinical features and postoperative recurrence after surgical excision in 130 patients with histologically confirmed perineal endometriosis treated at Peking Union Medical College Hospital between 1992 and 2020, using outpatient or telephone follow-up. Most patients reported symptom duration of 24–48 months before resection, and among 104 patients with completed follow-up the symptomatic recurrence rate was 15.1% (16 cases) over a median 99.0 months postoperatively; multiple lesions, microscopically positive margins, and anal sphincter involvement were independent recurrence predictors and were incorporated into a prognostic nomogram with good discrimination and calibration (C-index 0.84). The authors note a major limitation that the nomogram requires external datasets for further validation.
