This paper reports two cases of perianal endometriosis assessed with endoanal ultrasonography to guide operative management. The first patient had perianal pain, with ultrasonography showing a hypoechoic anterior perianal mass without anal sphincter involvement, and she underwent local excision without damage to the sphincter. The second patient’s perianal pain coincided with menstruation, and ultrasonography showed a heterogeneous mass with cystic anechoic areas in the right anterior perianal region involving the external anal sphincter; she received wide excision including part of the external anal sphincter and primary sphincteroplasty. The paper’s limitation is that the evidence is based on only two case reports, and the authors do not provide broader diagnostic performance estimates.
