Defining a minimal clinically important difference for endometriosis-associated pelvic pain measured on a visual analog scale: analyses of two placebo-controlled, randomized trials

This paper analyzed data from two placebo-controlled, randomized trials in 281 women with laparoscopically confirmed endometriosis and moderate-to-severe endometriosis-associated pelvic pain (EAPP) measured on a 100-mm visual analog scale (VAS). Using patients’ end-of-treatment satisfaction on a modified Clinical Global Impression (CGI) global improvement item as an anchor, the authors derived an empirically validated non-inferiority margin for clinically relevant changes in VAS pain and compared it with a distribution-based minimal clinically important difference approach (half standard deviation rule). They found anchor-based and distribution-based MCIDs of -7.8 mm and -8.6 mm, concluding that a 10 mm non-inferiority margin for EAPP on the VAS is appropriate. The study’s main caveat is that it relies on the CGI satisfaction anchor and includes methodological assumptions (e.g., handling missing data via physician replacement for one CGI item and last observation carried forward for missing VAS during dropout).
