This case report describes a 39-year-old woman with a history of abdominal wall endometriosis in a cesarean scar who developed recurrent masses with cyclic pain and later urinary symptoms, leading to imaging that showed invasion into the bladder and enlarged inguinal lymph nodes. After laparotomy with frozen-section confirmation of malignancy, extensive surgery was performed and pathology identified clear cell carcinoma arising in a background of endometriosis with widespread lymph node metastases (inguinal, pelvic, and para-aortic) and bladder involvement; the authors note that pre-surgical evaluation for lymph dissemination can be difficult, and in their cited prior cases it was often missed by imaging or exploration.
