Efficacy of hormonal therapies for decreasing uterine volume in patients with adenomyosis
Gonadotropin-releasing hormone agonist therapy significantly reduced uterine volume in adenomyosis patients, unlike low-dose estrogen/progestin or dienogest.

This retrospective cohort study evaluated 28 MRI-diagnosed adenomyosis patients who chose one of three hormonal therapies (GnRHa, low-dose estrogen–progestin [LEP], or dienogest [DNG]) for at least 16 weeks, assessing symptom outcomes and serum CA125 before and after treatment while using ultrasound to quantify uterine volume. CA125 decreased significantly in all groups, but uterine volume significantly decreased only with GnRHa (mean reduction), whereas LEP and DNG showed slight nonsignificant increases; the efficacy rate for inhibiting uterine-volume increase was much higher with GnRHa (96.2%) than with LEP (46.7%) or DNG (54.5%). The study’s key limitation is its small sample size, nonrandom treatment selection by patient preference, and reliance on a retrospective design and subjective symptom assessment.
