Long-Term Outcome after Laparoscopic Bowel Resections for Deep Infiltrating Endometriosis: A Single-Center Experience after 900 Cases

This single-center retrospective study evaluated long-term bowel and urinary dysfunction and fertility outcomes after laparoscopic bowel resections for deep infiltrating endometriosis (DIE) in 900 women (900/1023 eligible; discoid/transvaginal resections excluded), with telephone interviews and symptom questionnaires conducted 2011 and analyzed by pre-specified time periods. Key findings over a median 54-month follow-up included that most women reported resolution of postoperative tenesmus by follow-up, symptom rates varied by period for dyspareunia, constipation, diarrhea, pelvic pain, while dysuria and mild rectal bleeding did not significantly improve over time; pregnancies occurred in 16.5% (including 3.5% reporting at least one miscarriage) and relapse was reported in 2.9% with few requiring reoperation. Limitations included reliance on retrospective self-reported outcomes collected by telephone and incomplete long-term follow-up (774/900, 86%), which may bias estimates of long-term dysfunction and fertility.
