Salivary MicroRNA Signature for Diagnosis of Endometriosis

The prospective ENDO-miRNA study analyzed 200 women with chronic pelvic pain suggestive of endometriosis using small RNA sequencing of saliva to identify and validate a saliva-based microRNA (miRNA) diagnostic signature, with endometriosis status confirmed by laparoscopy with histology and/or MRI features (e.g., deep endometriosis and endometriomas), and controls defined as various benign pathologies or suspected cases without lesions found. Using DESeq2-based differential expression and a machine-learning workflow (feature selection plus a random forest classifier) with internal cross-validation across randomly generated datasets, the authors identified 109 miRNA candidates and reported diagnostic performance metrics (F1-score, sensitivity, specificity, and ROC AUC) ranging broadly across miRNAs.
