This paper reports a case of a 63-year-old woman initially evaluated for a suspected left inguinal hernia, ultimately found to have Stage IVB clear cell carcinoma arising from endometriosis in the left pelvic side wall with medial groin involvement and positive pelvic and inguinal lymph nodes. High-level management included neoadjuvant carboplatin plus paclitaxel followed by exploratory laparotomy with hysterectomy and bilateral salpingo-oophorectomy, extensive lymph node dissection, tumor debulking, and reconstruction, with final pathology also identifying a synchronous stage IA endometrial adenocarcinoma that was described as apparently unrelated. A biomarker workup showed the tumor was MSI stable, effectively excluding Lynch syndrome.
