This paper reports two cases of clear cell carcinoma arising from abdominal wall endometriosis in cesarean section scars, using clinical evaluation (ultrasound, CT, MRI), needle biopsy/cytology, and subsequent surgical pathology to establish origin and extent. In the first case, a patient with a long-ago cesarean scar developed an enlarging rectus muscle mass that biopsy was suspicious for clear cell carcinoma arising in an endometriosis setting, and complete excision followed by carboplatin/paclitaxel was complicated by later local recurrence responsive to radiation. The second case described a long history of pain with a large Pfannenstiel scar tumor and lymph node involvement where final pathology showed poorly differentiated Müllerian clear cell carcinoma with adjacent endometriosis lesions, managed with wide excision, hysterectomy with bilateral salpingo-oophorectomy, nodal surgery, and chemotherapy plus radiation. The authors emphasize that malignant transformation is rare with no specific diagnostic marker or standardized management, and decisions are based largely on limited case experience.
