Fertility Outcome after Operative Laparoscopy versus No Treatment in Infertile Women with Minimal or Mild Endometriosis.

This randomized clinical trial evaluated whether operative laparoscopy (ablation of visible implants) versus diagnostic laparoscopy alone improves clinical pregnancy outcomes in infertile Iranian women aged 20–32 with minimal or mild endometriosis, with normal semen analysis, ovulation, hormones, and hysterosalpingography; women were followed for 9 months and those receiving any hormonal therapy during follow-up were excluded. After confirmation of stage by Revised AFS criteria, 38 women per group were analyzed, and pregnancy occurred in 24% (9/38) in the operative group versus 18% (7/38) in the diagnostic-only group, which was not statistically significant (p=0.49); no surgical complications were reported, and anesthesia time was higher in the operative group. The authors’ main caveat was low statistical power due to the small sample size, along with increased loss to follow-up because participants could not always avoid non-protocol fertility/endometriosis medications. This paper is centrally about endometriosis — it tests whether operative laparoscopic ablation of minimal/mild endometriotic lesions improves fertility in infertile women.
