Characterization of Uterine NK Cells in Women with Infertility or Recurrent Pregnancy Loss and Associated Endometriosis
Women with unexplained recurrent pregnancy loss or infertility, with or without endometriosis, exhibited higher percentages of CD16+ uterine NK cells and higher ratios of NKp46+:CD56+ cells compared to fertile controls.

This study compared uterine endometrial expression of NK cell markers CD56, CD16, and NKp46 in eutopic endometrium from women with unexplained recurrent pregnancy loss (uRPL) or unexplained infertility (UI) versus fertile controls, and assessed how these measures differed by the presence of concomitant endometriosis. Using immunohistochemistry on endometrial biopsies taken in the window of implantation (LH plus 7–10 days) from 61 women, the authors found significantly higher proportions of CD16+ stromal cells in uRPL and UI groups and also in subfertile women with endometriosis compared with fertile controls, while CD56+ proportions showed a non-significant downward trend. NKp46+ proportions were significantly higher in uRPL but not in UI, and the NKp46+:CD56+ ratio was significantly increased in uRPL, UI, and in subfertile women with endometriosis. The analysis was limited by small sample sizes in some subgroup comparisons and by the reliance on marker percentages rather than direct NK cell functional assays. Relevance to endometriosis: endometriosis was diagnosed laparoscopically in 36 subfertile participants and the paper directly reports increased CD16+ uNK marker proportions and an elevated NKp46+:CD56+ ratio in the endometriosis-with-uRPL/UI subgroup.
