Polypoid endometriosis mimicking invasive cancer in an obese, postmenopausal tamoxifen user

This case report describes a 62-year-old postmenopausal woman with morbid obesity and 43 months of tamoxifen use who presented with gross hematuria and imaging findings concerning for gynecologic malignancy, including an ovarian mass, carcinomatosis, and a thickened endometrial stripe. After high suspicion for advanced cancer, she underwent diagnostic laparoscopy followed by laparotomy with posterior pelvic exenteration and ureteral resection to achieve cytoreduction, and final pathology identified polypoid endometriosis with extensive involvement of the uterus, ovaries, bowel/rectosigmoid, peritoneum, appendix, and tissue adjacent to the ureter, with differentials including low-grade mullerian tumor. The paper notes the major limitation that despite the ultimate benign diagnosis, invasive sampling sufficient to exclude malignancy preoperatively was difficult, and management likely could not have changed given disease distribution and concern for occult cancer.
